What Does It Mean to Begin a Journey of Pain Free Life?

Pain is defined by the International Association for the Study of Pain as “an unpleasant sensory and emotional experience associated with, or resembling that associated with, actual or potential tissue damage.” This definition, updated in 2020, acknowledges that pain is subjective and shaped by psychological, emotional, and social factors. Why Painkillers Are Not the Complete Answer

Pain medication plays an important role in acute pain management. However, relying solely on painkillers for chronic pain often fails to address the underlying causes. The CDC’s 2022 Clinical Practice Guideline for Prescribing Opioids for Pain recommends maximising the use of nonpharmacologic and nonopioid therapies as appropriate for the specific condition and patient. Many noninvasive nonpharmacologic approaches, including physical therapy, “can improve pain and function without risk for serious harms”.

The CDC explicitly states that “high-quality evidence exists that exercise therapy (a prominent modality in physical therapy) for back pain, fibromyalgia, and hip or knee osteoarthritis reduces pain and improves function immediately after treatment”. Importantly, “improvements are sustained for at least 2–6 months”. Active approaches that engage the patient “have somewhat more robust evidence for sustained improvements in pain and function than more passive treatments”.

A 2024 systematic review and meta-analysis examining the effectiveness of physiotherapy in chronic back pain management found that adherence to physiotherapy and patient satisfaction were good. Pain neuroscience education works by helping patients understand how pain and the nervous system function, ultimately helping them reconceptualise their pain experience. When patients understand how pain works—how the nervous system can become sensitised, how thoughts and emotions influence pain perception, and how movement and activity can help retrain the brain—they are better equipped to engage in their recovery.

Cost-Effectiveness of Physiotherapy

A systematic review of economic evaluations found that physiotherapy was cost-effective in most studies related to arthritis, chronic low back pain, and chronic neck pain. Physical therapy has been shown to be a cost-effective treatment compared to medication for the treatment of chronic low back pain. Research shows that virtual physical therapy can deliver a 1.8-times return on investment. These findings demonstrate that investing in physiotherapy is not only clinically effective but economically sound.

The Global Burden of Disease 2019 report identified low back pain as a significant issue within musculoskeletal conditions, accounting for 7.4% of years lived with disability.

In Kenya, the situation is equally concerning. A scoping review identified low back pain as one of the leading conditions contributing to functioning problems in Kenya. The Council is “a regulatory body established under an Act of Parliament … whose main function is to make provision for training and licensing of physiotherapists and to regulate their practice”. The Council has “a responsibility to ensure practitioners, physiotherapy clinics/facilities/firms comply with the set rules and standards”.

The Kenya Society of Physiotherapists has been a member of World Physiotherapy since 1982, aligning local practice with international standards. This regulatory framework ensures that patients receive care from qualified professionals who adhere to recognised standards of practice.
